A Q4 Care Manager not only investigates concerns to find what is happening and why, but we go beyond that; we work to reverse and stabilize the progression of symptoms. We detect subtle changes by investigating physical, emotional, cognitive, and functional health thus allowing insight into the causes and contributing factors surrounding these concerns. At Q4, we are experts at identifying what resources are needed and putting quality services into place. Advocacy

A Q4 Care Manager advocates for seniors and their families. We advocate for the senior with physicians and other medical providers. Appointments can be attended to ensure that the physician understands symptoms, what medications are being taken, what other providers are recommending, and what needs are present. Seniors’ needs and priorities are uppermost for us Our advocacy is directed at ensuring that the problem is understood and that choices are conveyed along with the associated risks and benefits.
