The disease does not have to control your life and you have the power to control your disease. Often, the result of disease and disability is limiting function. We may be limited in our ability to care for our home, our finances, or ourselves. This threatens our independence and autonomy. Many assume these changes must take place and nothing can be done to stop it or reverse the effects. At Q4, our Care Managers are trained to see just the opposite; we look at what is limiting you and focus on improving your function. Taking medications is often a mindless thing. We have problems, we got to the doctor, and we are prescribed medications. We take our medications faithfully, but often they don’t eliminate the problem and it is difficult for your doctor to determine whether drug interactions, time of day, or side effects are causing the problem. Physicians are plagued with questions concerning lingering symptoms, new symptoms, or interfering medications. It is complex process and your Q4 Care Manager will monitor for medication reactions, interactions, and side effects. Your Care Manager will work with your physician and pharmacist to resolve the problems and restore your health.
